To be honest, I've never played the game (Dragon Quest X), but I can assure you that it's not very important to understand what's going on.
We have a lovely fairy and her will to become an adventurer, but she has only her enthusiasm, and no real knowledge about how she can become a real hero.
Luckily (?) she meet a kind, and totally unselfish friend that is willing to help her reach the goal, instructing her on the ways to be a real adventurer. Her path will lead to a sort of initiation, which consist on being able to endure some rough sex. They're going to kill monsters, so they for sure can do something like that. This is what is told to a virgin dwarf (she's really a dwarf? dwarves can't be that cute, but she's short for sure) before giving her the big requisite for her adventuring career.
No wonders that our fairy friend is quite terrorized by such vision, but her fears are tranquilized by her guide, she'll have no need to endure such rape in a public place, but it's possible to choose a gentle partner in the comfort of privacy.
He arrange everything for her, out of pure generosity, and so she can meet a guy that ends up giving her three rapes at the price of one. But, I don't think that she's too much disappointed, as we see in the ending that I'm not going to spoiler ~
About the art. The art. I adore Hi-per pinch. His way to express all the intensity of rough, or violent sex is unique. I can feel the exaggerated stimulation, the rough intrusion as it should be in an hentai, where feelings of impossible intensity can exist. To me, his art is emblematic of the entire hentai concept.
Afterwords: The only obscure thing, that could be explained knowing the game, is who are those big ogre-oni guys with the spiked shoulders. But this ignorance won't spoil the fun of their bigness invading the little girls of the story.
On an unrelated, and pretty stupid, note I think that the ugliness of the dwarven girls explains everything about the fantasy race of dwarves. Why they live in the dark (so they won't see them), why they love battle so much (preferring an onorable death to such mating) and also why they venerate their ancestors (for having had the courage to actually mate).
